Step 1

Select your language preference in the top right corner.

Step 2

Select your city.

Step 3

Narrow your search by selecting the schedule, service, or payment method.

Care Categories

How To: Be Listed on this Website

Providers must be one of the following:

Federally Qualified Health Center (FQHC)
Community Service Board/Health Center
Nonprofit Hospital
Non-profit Community Health Center (CHC)
Medical Practices accepting Uninsured Patients

Providers must accept at least one of the following forms of payment:

Medicaid, Medicare, PeachCare for Kids®, Sliding Scale Fee, Self-Pay

If you need to make updates or edits to your listing, please email info@gwinnettcoalition.org.
