Use the Gwinnett Health Finder map to find healthcare providers near you that accept un/underinsured patients. If you do not find what you are looking for, please complete our survey by clicking on the ? in the circle at the top right corner of the map. Sharing more about what you need will help us expand our resources.

Step 1

Select your language preference in the top right corner. Information is provided in English with Spanish, Korean, Vietnamese, Cantonese, and Mandarin coming soon.

Step 2

Enter your zip code or city.

Step 3

Narrow your search by clicking the care category that best fits your need.

How To: Be Listed on this Website

Providers must be one of the following:

  • Federally Qualified Health Center (FQHC)
  • Community Service Board/Health Center
  • Nonprofit Hospital
  • Non-profit Community Health Center (CHC)
  • Medical Practices accepting Uninsured Patients

Providers must accept at least one of the following forms of payment:

Medicaid, Medicare, PeachCare for Kids®, Sliding Scale Fee, Self-Pay

If you qualify and wish to be added to the site, click on “Add New Resource” just above the map. Complete the form provided and click “Create.” Our admin team will review, verify you meet our criteria, and push your listing live within 24-hours.
